Record everything you consume (food, snacks, medication, vitamins, supplements, etc.).  Blank forms can be downloaded below or you can create your own form.   Make sure to record the date that food was consumed.  To save time you can check the appropriate column to confirm that you confirmed ingredient labels, restaurant food and supplements were milk free.
